WHO WILL OVERSEE MY DIALYSIS?

The patient’s current doctor provides oversight for their home hemodialysis care.

 

WILL I EVER NEED TO TRAVEL TO THE CLINIC

The dialysis equipment is a single use system and remains in the patient’s home.

Water sampling, other environmental culturing, maintenance, and servicing of the hemodialysis equipment are maintained in the patient’s home.

 

HOW WILL I RECIEVE MY SUPPLIES

Dialysis supplies are delivered monthly by caring and conscientious personnel.

 

WHAT ABOUT MY MEDICATIONS

Medications such as Heparin, Epogen, IV iron, antibiotics, etc., will be administered by the dialysis nurse as ordered by a doctor.

 

HOW WILL I WORK WITH MY SOCIAL WORKER OR DIETITIAN

The social worker and renal dietitian will schedule home visits periodically.

 

HOW WILL MY LABS BE DRAWN

Monthly and mid-month blood samples will be collected and sent to the laboratory from the patient’s home.

 

WHAT ABOUT MY PLAN OF CARE

Patient care conferences will be held monthly with you and your health care support team.  

 

WILL I NOTICE CHANGES IN MY ELECTRICITY OR WATER BILL

Electricity and water use are minimal, but there may be slight increases.
